- Cost: Each Opander unit retails for approximately $45–$60 (vs. $5–$10 for a BVM mask). Reuse is not recommended.
- Learning curve: Providers trained on King or Combitube may over-insert the Opander, leading to epiglottic downfolding.
- Lack of pediatric data: Most studies focus on adults >50 kg.
- Not for witnessed apnea with intact reflexes: In opioid overdose with agonal breathing, a BVM may still be preferable.
